While your membership covers the primary care services provided by Sage Family Medicine, it does not include specialty care, hospitalizations, surgeries, or emergency care. We strongly encourage patients to maintain health insurance or join a health share to help cover these and other medical expenses outside of the practice.

  • Adult (age 25-64): $95/mo

  • Senior (age 65+): $80/mo

  • Young Adult (age 18-24): $70/mo

  • Two adults in shared household: $175/mo

  • Child (age 10-17*): $50/mo each

    *younger children may be accepted on individual basis - just ask!

One-time Enrollment Fee upon joining: $100 per adult, $50 per child. Maximum $325/family
